How do I login?

How do I find my benefit documents?
The best way to access your benefit documents is through your benefits portal. This will be the same portal you logged in to enroll in your benefits.

2. Click on “Documents”
3. Select the plan document you wish to view

How do I find a provider?
If you are looking for a medical provider, contact a Care Coordinator at 435-275-4492. The Care Coordination team can help you find a provider that will work well with your benefits and can assist you in making appointments.

How do I find out what my benefits are?
You can review your benefits details by logging in to your portal, or you can reach out to Member Services and someone from our team will be happy to assist you.
How do I get a new ID card?
To request a replacement ID card, contact Member Services at member@planstin.com or 888-920-7526.
Can I make changes to my plan?
To make changes to your health plan outside of your annual renewal period, you must have had a qualifying life event (QLE), and informed us, within 60 days of the event.
Examples of qualifying life events (QLEs) include:
- Marriage
- Divorce
- Birth
